Finding Your Card Verification Request: Alternative Methods for Submitting Documentation

If you are unable to upload your documentation in the event of a card verification request, you must get the 24HOURFLEX CARD VERIFICATION REQUEST associated with your expense and mail it, along with your itemized receipt verifying the card transaction, to 24HourFlex.  Please follow these simple steps to find and download the Card Verification Request.

The 24HOURFLEX CARD VERIFICATION REQUEST page contains all of the information that we need to be able to identify you, and the claim you are filing.  It also provides the fax number, mailing address, and email address for submitting your documentation via alternative means.  Please note:  This sample can not be used for your substantiation.

How to Get Your CARD VERIFICATION REQUEST

Step 1:  You will first have to login to your account (How to Log Into Your Account) to gain access to your request letter.

Step 2:  Once you are in your account, go to the MESSAGE CENTER tab and click VIEW STATEMENTS. Then click the Card Verification Request notice that corresponds to the expense(s) you are wanting to substantiate.

Step 3:  Print or save the document, and use it as the cover page for your documentation.  Fax, email, or mail BOTH the Card Verification Request page AND your itemized receipt verifying the expense to 24HourFlex. The fax number, email address, and mailing address are all located on the Card Verification Request notice.
