How To File A Healthy Lifestyles Claim

Step 1: From your account HOME page click the FILE A CLAIM button.
Step 2: Begin creating the reimbursement by selecting the PAY FROM account and the PAY TO account. Click the NEXT button when you are ready to continue.
Step 3: UPLOAD VALID DOCUMENTATION and select Next to continue. Uploading your receipts is the ideal and most secure method for submitting documentation. Alternate methods for submitting receipts include email, fax, mail, and drop-off (all alternate methods require the printable claim confirmation).
Step 4: Enter all required information (* indicates required field). To trigger the Healthy Lifestyles account, you must select MISCELLANEOUS for the category, and HEALTHY LIFESTYLES for the type. Once all of the required fields are completed, you may click NEXT to continue.
Step 5: Submit the claim from the TRANSACTION SUMMARY page. From the TRANSACTION SUMMARY you will have the option of adding another , removing, updating/editing, and submitting the claim(s). You will be required to check the I HAVE READ, UNDERSTAND, AND AGREE TO THE TERMS AND CONDITIONS box before you can select SUBMIT.
Step 6: Congratulations! You have successfully filed your claim. If you were unable to upload the receipt, you may select CLAIM CONFIRMATION FORM to use with alternate submission methods. (See below for an example of the claim confirmation form, which must accompany all claim receipts submitted via fax, email, or mail.)
CLAIM CONFIRMATION FORM: These documents are generated each time you file a claim. Please include them as the cover page when faxing, emailing, or mailing in your receipts. You do not need this document if you are uploading receipts through your secure online portal.
Keep an eye on your account for important notifications. Pending reimbursements will appear in the MESSAGE CENTER on your HOME PAGE.
*Once the claim is submitted, you will have the option to print the claim confirmation page.
**If you do not have the ability to upload your receipts, please use the printable claim confirmation page as the fax cover page or accompanying attachment to an email (the fax number/email address is located on the printable confirmation page).
***Processing on new claims for reimbursement typically takes 1 to 2 business days.

How To Transfer Your HSA To 24HourFlex

Managing multiple HSA accounts can be done, but consolidating them will bring ease to your life. If you have an existing HSA with another custodian, you can easily transfer it to your HSA with 24HourFlex. If you are interested in simplifying your life and transferring your HSA to 24HourFlex, please follow the step-by-step instructions provided below.
Things to keep in mind:
- Contributions made to the existing HSA will still count towards the current tax year contribution limit.
- The transfer itself does not count towards the current tax year IRS contribution limit.
- IRS contribution limits for 2017: Individual limit $3,400.00 / Family limit $6,750.00
- IRS contribution limits for 2018: Individual limit $3,450 / Family limit $6,850
- The HSA transfer will direct the current custodian to send the balance to your HSA with 24HourFlex.
- Transfers typically take 3 to 4 weeks, depending on how quickly your form is processed.
- If you have funds invested in mutual funds or stocks, liquidating your account may take additional time. Please contact your HSA custodian for additional details.
Please follow the simple steps below to get started.
Step 1: Start by downloading a copy of the HSA Transfer Form: Individual.
Step 2: Fill out the form, sign it, make a copy, and send it to your old HSA provider.
Step 3: Please confirm receipt of the form with your former HSA provider, and make sure that they do not have any questions. On average, allow for 3-4 weeks for the transfer to complete.
Step 4: You’re done! Now all of your HSA funds can be managed in one place through your 24HourFlex Consumer Portal.
