Every time you submit documentation to verify your 24HourFlex Debit Card use, four pieces of information MUST be included:
- Provider Name
- Service(s) Received or Item(s) Purchased
- Date of Service
- Amount of Expense Incurred
If the information we receive from you does not include all of these details, we will send you a Request for More Information (RMI) (see sample email to right).
If you do not submit the missing information within 30 days, we will send you an Overdue Notice.
After 45 days, your card will be suspended and the transaction will be treated as an amount owed. Your card will remain suspended until the information is submitted or the amount owed is fully repaid.
